Integrating Push notifications to Alexa would be a very useful feature. They could be indicated by a blinking light ring for example. Then the user could ask what notifications he received.
Of course one has to ensure that only few notifications arrive per day so the experience is not destroyed.
One possible usecase: Letting Alexa remind the user of an appointment.

Very useful for reminding patients for medication

Out of curiosity, how would you propose managing the number and type of notifications? That seems like a non-trivial problem to solve, as it's more of a best practices or UX question than a technical question.

2 cents - some customized skill would be my method of choice. Speaking from a SMART HOME perspective. Hooking this into already implemented custom skills would be a FIRST step IMO. Currently I do have a "message queue" in a custom skill, now while this does not just PIPE up and PUSH out a message or notification immediately upon invocation of the custom skill - it responds with "you have X # of messages in your message queue" - if I wish to hear them I say " play message queue" Closest thing so far.
